Влияние ревматоидного артрита на риск сердечной недостаточности: механизмы, диагностика и подходы к лечению

Аннотация

Ревматоидный артрит ассоциирован с повышенным риском сердечной недостаточности, что обусловлено хроническим воспалением, играющим центральную роль в его патогенезе. У пациентов с ревматоидным артритом дисфункция миокарда может оставаться субклинической, в то время как сердечная недостаточность часто представлена в форме с сохраняющейся фракцией выброса. В статье рассматриваются основные механизмы возникновения сердечной недостаточности у пациентов с ревматоидным артритом, а также диагностические и терапевтические подходы. Существуют значительные проблемы с диагностикой сердечной недостаточности, особенно на фоне внедрения современных методов лечения, способных улучшить исходы заболевания и качество жизни. Раннее и целенаправленное лечение воспалительных процессов имеет первостепенное значение. Однако требуется дальнейшее исследование для понимания биологических механизмов, лежащих в основе этой взаимосвязи, а также для оценки стратегий лечения. Будущие исследования могут выявить биомаркеры, предсказывающие риск сердечной недостаточности, а также оценить эффективность противовоспалительных вмешательств в профилактике и лечении этой патологии у пациентов с ревматоидным артритом.
