Abstract
The critical role of insulin resistance (IR), closely associated with abdominal obesity (AO), in the initiation and progression of cardiometabolic risk (CMR) during perimenopause necessitates studying the associations of waist circumference (WC), a clinical marker of AO, with parameters of menopausal metabolic syndrome (MetS).
Aim. To study the relationships between waist circumference, the TyG index, MetS characteristics, and the hormonal chronology of perimenopause in a cohort of women with different blood pressure (BP) levels.
Materials and methods. Of the 88 women aged 35–59 years, 58 had hypertension and 30 were normotensive. The following parameters were determined: WC, blood pressure, triglycerides (TG), HDL-C, insulin, follicle-stimulating hormone (FSH), estradiol, glucose, TyG index, and postmenopausal duration (PMD). The following parameters were assessed (SPSS, version 23): median [25th; 75th percentiles]; intergroup differences using the Mann-Whitney test; comparison of proportions using the Pearson χ2 test; Spearman correlation (R) and partial correlation (Rрс) analyses were performed to adjust for the influence of age.
Results. Statistically significant associations of WC were identified: negative with estradiol and HDL-C levels, positive with PMD, TyG, glucose, TG, blood pressure, and insulin levels, with no associations with FSH. WC also showed a Spearman correlation with the TyG index (R=0.526; p<0.001), which correlated with age-dependent FSH (R=0.212; p<0.05) and independently with age-dependent estradiol (R=0.252; p<0.01) and PMD (R=0.338; p<0.001). FSH and estradiol levels correlated with MetS markers completely in an age-dependent manner. The associations of PMD with FSH and TyG were more stable, persisting after age adjustment.
Conclusions. Correlations of perimenopausal parameters clarify their influence on MetS components, with the additional effect of PMD as a parameter of reproductive aging in addition to chronological aging. The OT association complex, reflecting the importance of anthropometric indicators as predictors of metabolic disorders, clarifies the factors that shape OT during perimenopause, drawing attention to its early stages for timely preventive interventions.
