Epidemiology of actinic keratosis and non-melanoma skin cancer in the elderly in Russia: a literature review

Abstract

The incidence of actinic keratosis (AK) and non-melanoma skin tumors (NMST) among the elderly in Russia shows a steady increase. In 2022, 79 399 new cases of NMST were registered, of which 29 686 were in men and 49,713 in women. The average age of patients was 69.7 years, which confirms the tendency for the incidence to increase with age. The cumulative risk of developing NMST for the period 2012–2022 for individuals under 74 years of age was 3.03%. At the same time, actinic keratosis is considered a predictor of NMST development, especially squamous cell skin cancer. The distribution of diseases is characterized by significant regional differences, which is associated with variability in the intensity of ultraviolet radiation, socio-economic conditions and features of medical care in different regions of the country. The current methodological heterogeneity of studies and the lack of standardization of registration systems complicate accurate assessment of prevalence, make it difficult to identify demographic and clinical risk factors, and limit the possibilities for comparative analysis. The main risk factors are chronic exposure to ultraviolet radiation, fair skin phototype, history of sunburn, and concomitant chronic diseases such as immunodeficiency states and diabetes mellitus. Insufficient knowledge of the effectiveness of preventive and screening measures in older people highlights the need to develop targeted strategies for early detection and prevention. These data allow us to form a comprehensive understanding of the dynamics of incidence, risks, and needs for the management of NMOSD in the elderly in Russia, and also identify priority areas for further research.
