Clinical case of dermatomyosits in a patient with polycythemia vera and postpolychytemic myelofibrosis

Abstract

Polymyositis and dermatomyositis belong to the group of inflammatory myopathies, presenting with proximal muscular weakness and inflammatory changes of the muscles; dermatomyositis in addition characterized by different types of skin rush. Laboratory findings include elevated muscle enzymes levels, most of patients have myositis specific or myositis associated autoantibodies, or non-specific antinuclear antibodies. Association of dermatomyositis/polymyositis and cancer, mostly adenocarcinomas, is well established, while association with blood malignancies, such as non-Hodgkin lymphomas, myelodysplastic syndrome or primary myelofobrosis is much rarer. We found just a single case of dermatomyositis, associated with polycythemia vera and postpolychytemic myelofibrosis. Hereby we present a second to our knowledge case of a patient with polycythemia vera, postpolychytemic myelofibrosis and dermatomyositis, and discuss the difficulties of myopathy differential diagnostics.
