Abstract

Background. Traumatic brachial plexopathy is characterized by persistent motor deficit, neuropathic pain syndrome, and significantly decreased quality of life. Standard therapeutic approaches often ensure slow and incomplete functional recovery, what determines the need to search for additional pathogenetically substantiated treatment methods.
Aim. To assess efficacy and safety of the Cellex® drug inclusion in the combination therapy regimen for traumatic brachial plexopathy.
Methods. A prospective open-label comparative study of 59 patients with the verified diagnosis of traumatic brachial plexopathy was conducted. All the patients were simply randomized into the index group receiving the Cellex® drug and the control group not receiving the drug. Patients in both samples received adequate therapy (analgesics, B vitamins) and the standardized set of therapeutic exercises. The follow-up period was 56 days. Assessment was performed based on the MRC scale, wrist dynamometry data, goniometric measurements of active and passive motions, visual analogue scale, DN4, HADS scales, Pain Catastrophizing Scale, Leeds Sleep Evaluation Questionnaire scores, as well as electroneuromyography data.
Results. The earlier and more pronounced muscle power restoration, primarily in the upper limb distal parts, was reported in the index group, along with the increase in active motion range, as well as more significant decrease in pain intensity, including neuropathic component. The anxiety, depression, sleep disturbance, and pain catastrophizing scores significantly decreased against the background of Cellex therapy. The increase in the amplitude of motor and sensory responses without significant changes in speed indicators was revealed based on electroneuromyography data. No adverse events were recorded.
Conclusion. The Cellex® inclusion in the combination therapy regimen for traumatic brachial plexopathy contributes to faster functional recovery, pain syndrome relief, and patient psychoemotional state improvement, with the beneficial safety profile.
Keywords: traumatic brachial plexopathy, neurotrophic therapy, neuropathic pain, electroneuromyography, Cellex.
