Abstract

Pancreatic schwannoma is an extremely rare benign neurogenic tumor arising from Schwann cells of peripheral nerve sheaths within the pancreas. Preoperative diagnosis remains challenging because of the absence of specific clinical and radiological features, while the frequent cystic-solid appearance of the lesion may mimic mucinous and other cystic pancreatic neoplasms.
Objective: to present a clinical case of pancreatic schwannoma mimicking a mucinous pancreatic neoplasm and to analyze the radiological and morphological diagnostic features of this rare tumor. A 66-year-old female patient was evaluated for a cystic-solid lesion of the pancreatic isthmus measuring up to 30 mm. Preoperative cytological examination suggested pancreatic adenocarcinoma. MRI and CT demonstrated a well-circumscribed lesion without evidence of vascular invasion or pancreatic duct dilation. The patient underwent pylorus-preserving pancreaticoduodenectomy. Definitive diagnosis was established by histopathological and immunohistochemical examination of the surgical specimen and revealed pancreatic schwannoma with degenerative changes. Morphologically, the tumor demonstrated a characteristic biphasic pattern with Antoni A and Antoni B areas, cystic degeneration, calcification, and diffuse S-100 expression. Pancreatic schwannoma should be included in the differential diagnosis of cystic-solid pancreatic lesions. The present case highlights the complexity of preoperative diagnosis of rare neurogenic pancreatic tumors and emphasizes the importance of comprehensive interpretation of imaging, cytological, and pathological findings in order to optimize surgical strategy and avoid unnecessary extended pancreatic resections.
Keywords: pancreatic schwannoma, pancreas, cystic-solid tumor, morphological diagnosis, immunohistochemistry, S-100, differential diagnosis, pancreaticoduodenectomy.
